*We are actively seeking to hire a compassionate and experienced RN Case Manager.
*
*Responsibilities include:*
* Responsible for observation, assessment, nursing diagnosis, counseling, care and health teaching of the ill, injured or infirm, and the maintenance of health and prevention of illness of others.
* Complete initial nursing assessment within 24 to 48 hours (per state regulations) of client admission and completes care plan.
Establishes nursing interventions to achieve care plan.
* Explain and/or provide counseling of all procedures, treatments, and medications to client and family, as appropriate.
* Review client status and care provided and chart progress notes.
Evaluate and revise nursing interventions, as appropriate, and provide progress notes to the client's physician or podiatrist about clients when the client's conditions change or there is a deviation from the plan of care or at leave every sixty days or as otherwise required by state regulation.
* Direct the activities of clinical care to LPN's, CNA's and other ancillary staff.
Prepare written instructions for the CNA and provide supervision of the CNA in the home, as per state/federal regulations.
Provide leadership and management to the LPN's, CNA's and on-call nurses.
* Administer medications and treatment as prescribed by the client's physician.
